Optical and Dental Plan Providers Dental Plan providers: IMPORTANT MESSAGE -- PLEASE READ: The list of participating dentists is kept as current as possible as of the month and year indicated. However, there are literally hundreds of dentists on the panel, and participating dentists are continuously being added to and dropping off the list. Therefore, the list is merely provided for your convenience, but cannot be relied upon to determine whether an individual dentist is participating or not at any given time. If you wish to be certain that you are using a participating dentist, it is YOUR RESPONSIBILITY to ask your dentist BEFORE EACH AND EVERY VISIT whether he is still on the Fund's panel. |
